The following items must be submitted (for translation of Identity Card, Birth Certificate, Driving License and Marriage Certificate)
1. Certified Copy (by local JP or Notary Public) of Malaysian Document.
2. Translated Copy of Malaysian Document (by the official translator).
3. Fee per document payable in bank cheque to Malaysian High Commission. Personal cheque or cash will not be accepted.
4. Prepaid self addressed courier bag.
5. If to be present at the High Commission, please make an appointment with the Consular Section.
NOTE: 
A. Please abide with the above checklist when submitting your application. An incomplete application will be returned.
B. Limited translation services of Identity Card, Driving License and Birth Certificate can be processed by the High Commission. For translation of other documents, kindly process your translation of document with the official translation services before submitting for certification by the High Commission.
C. IMPORTANT: High Commission of Malaysia will not accept any responsibilities for documents loss during to and from High Commission via post or any courier company.
